​AUTHORIZATION FOR DONATION

​Every donation begins with the selfless decision to help others. LifeShare works with families to ensure that the opportunity for donation is offered to those eligible.
​

​FIRST PERSON AUTHORIZATION

​Many individuals have already registered their decision to become donors on the Donate Life Registry. In these cases, the donation has already been authorized by the patient prior to the time of death. The family is given a copy of the Document of Gift form, which informs the family that their loved one is a registered donor, along with proper documentation of the gift.

DONATION AUTHORIZED BY PATIENT'S FAMILY

​For individuals who are not already registered as donors, their legal next-of-kin must make that decision for them when the LifeShare staff member seeks authorization for donation. Once the donation is authorized, the next-of-kin signs an authorization form and a copy of the form is provided to them.
